Job Overview: 

Our client, a sector leader recognized for over two decades of excellence, is seeking a Registered Nurse (RN) with a passion for hospice care to provide compassionate services in Northbrook, IL. This role requires a professional who can deliver end-of-life care with dignity and respect, ensuring comfort for both patients and their families.

 

Responsibilities:

- Provide superior hospice care to geriatric patients in their homes, focusing on pain management and symptom relief.

- Develop and oversee the execution of personalized care plans in collaboration with patients, families, and other healthcare professionals.

- Administer medications and treatments as prescribed and monitor patient response.

- Ensure accurate documentation of patient care services and compliance with federal, state, and local regulations.

- Educate patients and families on end-of-life care and support systems available.

- Participate in a comprehensive 6-week onboarding program to align with the company’s high standards of care.

- Travel locally to patient homes with mileage reimbursement and potential participation in the company vehicle program after 6 months.

 

Requirements:

- Active RN License in the State of Illinois.

- Associate’s Degree in Nursing or higher.

- At least one year of professional experience as an RN, with hospice care experience preferred.

- Current BLS and CPR Certification; training provided if necessary.

- Valid Driver's License, reliable mode of transportation, and auto insurance.

- Strong interpersonal and communication skills, with the ability to provide empathetic care to patients and families.
