Job Description: We are seeking a Hospice Admissions Registered Nurse to join our team, a team that has been a leader in pioneering business and technological solutions within home health and hospice fields for over two decades. Qualifications You Would Need:

- You should hold an Associate degree or a higher-level qualification, with experience of over 2 years in Hospice and a year with TruChart EHR or any Electronic Medical Record (EMR) system.

- You must possess a valid Registered Nurse (RN) certification/license in Pennsylvania (PA), and you must have a valid driver's license, a reliable mode of transportation, and automotive insurance.

As a Hospice Admissions Registered Nurse, you will become an integral part of our extraordinary care team, contributing to crafting patient care plans and providing support to patients. Your unwavering commitment to delivering quality care will shine through in all your interactions.

Key Duties:

- Collaborate in the design and execution of personalized patient care plans.

- Support patients in Mt. Lebanon, PA, and surrounding communities.

- Adhere strictly to charting deadlines.

- Manage personal work time and schedule.

- Demonstrate aptitude with tablets, mobile phones, and EMR software.

Schedule and Shift Details:

This position entails working 3 - 12 hour shifts on a revolving schedule of 10 am - 10 pm, managing around 3 admissions per day. Shift patterns alternate weekly between Monday, Tuesday, Wednesday, and Wednesday, Thursday, and Friday.
