Job Summary

We are looking for individuals to join our client’s team as Case Worker. Under supervision, social services are provided to clients/patients, such as recipients of public assistance, food stamps, and/or medical assistance, and adults receiving or requiring institutional care or protective services.

 

Job Description

  • Conduct interviews, complete initial assessments, and make appropriate referrals.
  • Manage ongoing cases of social service recipients.
  • Conduct home visits to assess client needs, living conditions, and provide support as needed.
  • Perform referral management, including coordinating resources and ensuring clients are linked to appropriate services.
  • Collect, verify, and investigate information obtained from clients and other sources.
  • Provide coordination of health-related and social services for clients.
  • Assist social workers with discharge planning and follow-up care coordination.
  • Develop, implement, and update social service plans for individuals and families, including assisting in determining eligibility for services and providing supportive counseling and related casework functions.

 

Skills Required

  • Minimum of 3 years’ experience in social services, case management, or related field.

 

Education/Training/Certifications

  • Bachelor’s degree from an accredited college or university required.

 

Additional Requirements

  • Shift Time: 9:00 AM-5:00 PM

 

 Pay Rate

  • $37–$43 per hour, based on experience, skills, and market factors
