Job Summary

We are looking for individuals to join our client’s team as Case Worker.  Under supervision, provides social services to clients/patients, including recipients of public assistance, food stamps, and/or medical assistance, as well as adults receiving or requiring institutional care or protective services. Identifies, develops, and implements social service plans for families and individuals, determines eligibility for services, and may perform counseling and investigatory activities as needed.

 

Job Description

  • Conduct interviews and comprehensive assessments of clients’ social, emotional, and medical needs.
  • Develop and implement individualized social service plans to support clients and families.
  • Determine eligibility for public assistance, food stamps, medical assistance, and other supportive services.
  • Provide counseling and guidance to clients to address social and personal challenges.
  • Perform investigatory activities related to client needs, eligibility, and compliance with program requirements.
  • Connect clients with community resources, including housing, financial assistance, and healthcare programs.
  • Collaborate with healthcare providers, social workers, and other professionals to ensure coordinated care.
  • Maintain accurate and timely documentation of client interactions, assessments, and progress reports in compliance with organizational and regulatory standards.
  • Participate in team meetings, training sessions, and professional development activities.

 

Skills Required

  • Minimum of 3 years’ experience in social services, case management, or healthcare support.
  • Knowledge of social service programs, public assistance, and institutional care services.
  • Strong interpersonal, communication, and problem-solving skills.
  • Ability to perform assessments, counseling, and investigatory work with attention to detail.
  • Ability to work independently and as part of a multidisciplinary team.

 

Education/Training/Certifications

  • Bachelor’s Degree required from an accredited college or university

 

Additional Requirements

  • Shift Time:     8:00 AM-4:00 PM

 

 Pay Rate

  • Up to $43.00 per hour depending on experience
